First-in-human studies. Safety, tolerability, and pharmacokinetics of injectable semaglutide established. Half-life of ~7 days confirmed — enabling once-weekly dosing.
SUSTAIN 1–6: semaglutide vs placebo, sitagliptin, exenatide, insulin glargine, and dulaglutide. All primary HbA1c endpoints met. SUSTAIN-6 showed 26% MACE reduction.
FDA approved injectable semaglutide (Ozempic) for type 2 diabetes. First once-weekly GLP-1 agonist with demonstrated cardiovascular benefit in T2D.
PIONEER 1–10: oral semaglutide (Rybelsus) vs placebo, sitagliptin, empagliflozin, liraglutide, and insulin. All primary HbA1c endpoints met.
FDA approved oral semaglutide (Rybelsus) for type 2 diabetes — the first oral GLP-1 receptor agonist ever approved.
STEP 1–4: semaglutide 2.4 mg vs placebo in adults with obesity. STEP 1 showed 14.9% mean weight loss at 68 weeks. STEP 4 showed 7.9% additional loss with continued treatment.
FDA approved semaglutide 2.4 mg (Wegovy) for chronic weight management — the first new obesity drug approved in 8 years and the first GLP-1 agonist approved specifically for obesity.
FDA expanded Ozempic's label to include cardiovascular risk reduction in T2D patients with established CVD, based on SUSTAIN-6 and LEADER-equivalent data.
SELECT trial (n=17,604): Wegovy reduced MACE by 20% in adults with obesity and CVD but without diabetes — first obesity drug to show CV benefit in a non-diabetic population.
FDA approved Wegovy for cardiovascular risk reduction in adults with obesity or overweight and established CVD — the first obesity drug approved for CV benefit.
FDA approved Ozempic to reduce the risk of sustained eGFR decline, end-stage kidney disease and cardiovascular death in adults with type 2 diabetes and chronic kidney disease, based on the FLOW trial.
Accelerated approval of Wegovy for MASH with moderate-to-advanced liver fibrosis. Week-72 interim analysis (n=800): MASH resolution without worsening fibrosis in 63% vs 34% on placebo. The trial continues to 240 weeks to confirm outcomes.
First oral GLP-1 approved for weight management, based on the OASIS program (OASIS 4: about 14% weight loss over 64 weeks counting all participants) and SELECT. US launch January 2026.
Higher 7.2 mg Wegovy dose approved for weight loss and maintenance, 54 days after filing under the FDA's National Priority Voucher pilot.
The SELECT trial (NCT03574597) was a landmark cardiovascular outcomes trial that enrolled 17,604 adults with overweight or obesity (BMI ≥27) and established cardiovascular disease, but without diabetes. Participants were randomized to semaglutide 2.4 mg once weekly or placebo for a median of 34.2 months. The primary endpoint was time to first occurrence of a major adverse cardiovascular event (MACE): cardiovascular death, non-fatal myocardial infarction, or non-fatal stroke.
SELECT results, published in the New England Journal of Medicine in November 2023, showed a 20% reduction in MACE with semaglutide vs placebo (HR 0.80, 95% CI 0.72–0.90, p<0.001). This was the first time an obesity drug had demonstrated cardiovascular benefit in a non-diabetic population — a historic milestone that led to the March 2024 FDA label expansion for Wegovy. The SELECT trial also showed significant reductions in all-cause mortality, heart failure hospitalizations, and kidney function decline.
Approved as the Wegovy pill (25 mg once daily), the first oral GLP-1 for weight management.
Approved for Ozempic: lower risk of sustained eGFR decline, end-stage kidney disease and cardiovascular death in adults with type 2 diabetes and CKD.
Approved for Wegovy on week-72 histology; continued approval depends on the 240-week outcome results.
The two Phase 3 trials (3,808 adults) did not show slower progression versus placebo on CDR-SB. No FDA filing; Novo Nordisk ended the programme.

Yes. Semaglutide has four FDA-approved brand names: Ozempic (injectable, approved December 2017 for type 2 diabetes), Rybelsus (oral tablet, approved September 2019 for type 2 diabetes), Wegovy (injectable, approved June 2021 for chronic weight management), and Ozempic with a cardiovascular risk reduction indication (approved March 2023). All are manufactured by Novo Nordisk. Ozempic and Wegovy contain the same active ingredient at different doses — Ozempic up to 2 mg, Wegovy up to 2.4 mg.
Semaglutide (Ozempic) was first FDA approved on December 5, 2017, for adults with type 2 diabetes. Semaglutide (Rybelsus) received oral tablet approval on September 20, 2019. Semaglutide (Wegovy) was approved on June 4, 2021, for chronic weight management. In March 2023, Ozempic received an expanded label for cardiovascular risk reduction in adults with T2D and established cardiovascular disease — the first GLP-1 agonist approved for this indication.
Ozempic and Wegovy both contain semaglutide but differ in FDA-approved indication and maximum dose. Ozempic (0.5 mg, 1 mg, 2 mg) is approved for type 2 diabetes management and cardiovascular risk reduction. Wegovy (0.25 mg, 0.5 mg, 1 mg, 1.7 mg, 2.4 mg) is approved for chronic weight management in adults with obesity or overweight with a weight-related condition. Wegovy's 2.4 mg dose produces ~15% mean weight loss vs ~6–8% at Ozempic's 1 mg dose, reflecting the dose-dependent nature of semaglutide's weight effects.
Rybelsus is the oral tablet formulation of semaglutide, approved September 2019 for type 2 diabetes. It uses a novel absorption enhancer (SNAC — sodium N-(8-[2-hydroxybenzoyl]amino)caprylate) to enable GI absorption of the peptide. Rybelsus doses (3 mg, 7 mg, 14 mg) are lower than injectable Ozempic due to oral bioavailability (~1% vs ~89% for subcutaneous injection). Rybelsus is the first oral GLP-1 receptor agonist approved by the FDA. It is not approved for weight management.
The SELECT trial (Semaglutide Effects on Cardiovascular Outcomes in People with Overweight or Obesity) enrolled 17,604 adults with overweight/obesity and established cardiovascular disease but without diabetes. Published in the New England Journal of Medicine in November 2023, SELECT showed semaglutide 2.4 mg (Wegovy) reduced major adverse cardiovascular events (MACE) by 20% vs placebo (HR 0.80, 95% CI 0.72–0.90, p<0.001). This led to an expanded Wegovy label for cardiovascular risk reduction in March 2024 — the first obesity drug approved for CV benefit.
Semaglutide (Ozempic/Wegovy) was FDA approved for T2D in 2017 and obesity in 2021 — the most established approval history of the three agents. Tirzepatide (Mounjaro/Zepbound) was approved for T2D in 2022 and obesity in 2023. Retatrutide is not FDA approved; its Phase 3 trials have reported and Lilly plans to file in Q1 2027. Semaglutide produces ~15% weight loss (STEP 1), tirzepatide ~22.5% (SURMOUNT-1), and retatrutide ~24% (Phase 2). Semaglutide has the most long-term safety and cardiovascular outcomes data of the three.
